The Health Links approach to Coordinated Care Planning benefits those individuals with multiple health care needs, who may see many different providers, by bringing a Care Team together to help develop a plan based on what is most important to the patient/client. It is a philosophy of care with the goal of improving the individual's well-being while making our health system more efficient.
This day of learning will focus on developing skills to further enhance everyone's comfort level to participate in and/or lead coordinated care planning. The day will include how to identify and engage individuals who would benefit from the Health Links approach to coordinated Care Planning. In addition, there will be real case scenarios for participants to practice interviewing patient/clients and facilitating/participating in a care conference.
